We are hiring a Virtual RN – Intake Coordinator!
Key Responsibilities
- Conduct comprehensive intake assessments for new patients/residents via phone or video conferencing.
- Gather medical history, current health status, and care needs to determine eligibility and appropriate level of care.
- Develop individualized care plans in collaboration with healthcare providers, patients, and their families.
- Ensure that care plans meet the specific needs of each patient and comply with regulatory standards.
- Serve as the primary point of contact for new admissions, providing information and support to patients, families, and referral sources
- Coordinate with healthcare providers, social workers, and other team members to facilitate a smooth admission process.
- Maintain accurate and up-to-date electronic health records for all new admissions.
- Ensure all documentation is completed promptly and meets regulatory and organizational standards.
- Ensure all intake processes comply with relevant healthcare regulations, including HIPAA, and assisted living standards.
- Stay informed about changes in regulations and update procedures as necessary.
- Provide education and resources to patients and families about the assisted living facility’s services, policies, and procedures.
- Offer guidance on available financial resources and assist with completing necessary paperwork.
- Monitor and evaluate the intake process to ensure high-quality care and service delivery.
- Conduct follow-up calls or virtual check-ins with newly admitted patients to ensure their needs are being met and to address any concerns.
- Coordinate ongoing care and services as needed to ensure continuity of care.
Qualifications
- Must be a licensed Registered Nurse in the Philippines, with a robust background in assisted living, medication administration, and healthcare technology.
- Demonstrated experience in EMAR software, particularly Chartmed, with the ability to identify, troubleshoot, and resolve discrepancies in medication management records.
- Previous experience in intake roles, managing patient admissions, assessments, and coordinating care plans.
- Proficient in conducting clinical assessments and evaluating patient needs to determine appropriate care levels.
- Strong verbal and written communication skills for interacting with patients, families, healthcare providers, and team members. Proficiency in telehealth communication tools is essential.
- Proficient in technology, with a strong aptitude for learning and utilizing new platforms such as Trainual, Google Meet, and Zoom for training and communication purposes.
- Strong organizational and problem-solving skills, with a keen eye for detail and the ability to manage multiple tasks efficiently.
- Flexible schedule availability to accommodate different time zones, primarily aligned with California business hours, and to respond to urgent care concerns promptly.
