– Conduct screener questions with patients over the phone.

– Utilize EMR (Electronic Medical Records) systems effectively.

– Work collaboratively to ensure smooth care coordination.

– Proficiency in Spanish and English.

– Experience with EMR systems.

– Flexibility to work between 16-38 hours per week in the EST time zone.

– Strong communication and organizational skills.
